EMERGENCY MEDICAL INFORMATION NOTEBOOK


In an effort to assist our residents in being prepared for a medical emergency and to receive proper care, we have a prepared Emergency Medical Information Notebook. This notebook has forms for you to fill out that will assist emergency personnel, your doctor, and/or a hospital in your care during an emergency when you are unable to speak for yourself or remember important information regarding your health history or medications.

This notebook also includes forms from the Texas Department of State Health Services to make known for your care. With all legal forms, please consult your family, doctor, caregiver and/or attorney. This is not intended to give you legal advice nor are you required to use the following forms.

1. Advance Directive/Living Will - is designed to help you communicate your wishes about medical treatment at some time in the future when you are unable to make your wishes known because of illness or injury.

2. Medical Power of Attorney - Except to the extent you state otherwise, this document gives the person you name as your agent the authority to make any and all health care decisions for you in accordance with your wishes, including your religious and moral beliefs, when you are no longer capable of making them yourself.

3. Out-of-Hospital Do-Not-Resuscitate Order - This form instructs emergency medical personnel and other health care professionals to forgo resuscitation attempts and to permit the patient to have a natural death with peace and dignity. This order does NOT affect the provision of other emergency care including comfort care.

4. Statutory Durable Power of Attorney - This form is for designating an agent who is empowered to take certain actions regarding your property. It does not authorize anyone to make medical and other healthcare decisions for you.

This notebook includes information on CodeRed which Erath County uses to notify residents in an emergency. If you need assistance with registering, please ask for assistance.

We have included information on STEAR - State of Texas Emergency Assistance Registry- This will allow emergency management to plan for persons during an emergency or disaster for possible transportation and needs.

We hope the information will speak for you when you are unable and to ensure you get information as needed from Erath County.
